CBHA offers Spravato (esketamine) for adults with treatment-resistant depression or MDD with suicidal thoughts or behaviors.

What Is Spravato?

Spravato is an FDA-approved nasal spray given only in a certified clinic with two hours of on-site monitoring.

Unlike traditional oral antidepressants that target neurotransmitters like serotonin, Spravato works on the glutamate system, offering a novel approach to relieving depressive symptoms. It is always prescribed alongside an oral antidepressant, providing relief even when previous medications have been ineffective.

Most people begin twice-weekly sessions and taper to maintenance as symptoms improve.

Spravato is FDA-approved for adults who meet one of two criteria:

  • Have tried two or more oral antidepressants without adequate improvement (treatment-resistant depression)
  • Experience depressive symptoms accompanied by suicidal thoughts or behaviors (MDD with suicidality)

Spravato is not a first-line treatment for depression. It is used when conventional medications and therapies have not provided sufficient relief.

What to Expect During Treatment

Spravato is administered exclusively in a certified medical setting under close supervision. You do not take this medication at home. Instead, you’ll visit one of our REMS-certified treatment centers, where your care team will guide you through every step of the process.

Treatment visits follow this general structure:

  1. Arrival and pre-treatment check-in: Our clinical staff will take your vitals, review your symptoms, and ensure you are medically cleared to receive the day’s dose.
  2. Self-administration: You will use the nasal spray under the guidance of a licensed provider.
  3. Observation period: After taking Spravato, you will remain in the office for two hours so we can monitor your response and check for any side effects.
  4. Discharge: You must arrange for a responsible adult to drive you home. You will not be permitted to drive or operate machinery for the rest of the day.

Some patients notice mood improvement within their first few treatments, while others experience gradual progress over several weeks. Your provider will work with you to determine the appropriate schedule and evaluate progress along the way.

Is Spravato Right for You?

Spravato may be an option if you are an adult who has not responded to at least two different oral antidepressants, or if you are currently experiencing suicidal thoughts or behaviors in the context of a depressive episode. To determine if you are eligible, our team will conduct a comprehensive psychiatric evaluation that includes your medical history, current medications, symptom profile, and past treatment attempts.

To receive Spravato, you must meet the following requirements:

  • Be over the age of 18
  • Have a documented diagnosis of treatment-resistant depression or MDD with suicidal ideation
  • Be taking an oral antidepressant concurrently
  • Be medically cleared for Spravato treatment through a REMS (Risk Evaluation and Mitigation Strategy) certified provider and facility

Spravato is not recommended for individuals with a history of substance misuse involving ketamine or similar medications, or for those with certain medical conditions. We will discuss all risks and benefits with you before treatment begins.

Appointments are available throughout the week and are scheduled based on your treatment plan. You will likely begin with twice-weekly sessions, transitioning to weekly or less frequent maintenance doses as you improve.
